Kami akan meminta anda menandatangani surat kuasa untuk memungkinkan rekan kerja anda memberikan informasi tersebut. Sementara itu, kuesioner bertujuan untuk meminta anda memberikan ranking pada sejumlah aspek berdasarkan frekuensi, tingkat kesulitan, efisiensi, atau kemungkinan dari sejumlah hal yang telah diperoleh dari wawancara tahap pertama.
Setelah proses ini, kami menganalisis hasil isian kuesioner dan membuat statistic dasar untuk setiap jawaban. Kami kemudian menghubungi anda Kembali untuk wawancara tahap ketiga. Tujuan wawancara tahap ketiga adalah meminta anda meninjau hasil isian kolektif dari kuesioner, mendengarkan komentar dari anda tentang hasil tersebut, serta meminta anda mempertimbangkan untuk mengubah atau tidak mengubah jawaban anda terhadap kuesioner tersebut setelah melihat hasil isian kolektif tersebut. Jika anda juga merekomendasikan seseorang sebagai rekankerja pada wawancara tahap kedua, maka pada tahap ini kami akan melakukan wawancara pula pada rekan kerja tersebut.
Setiap wawancara dan pengisian kuesioner hanya akan memakan waktu satu jam.
Karenanya, secara total, partisipasi anda secara langsung dalam penelitian ini adalah tiga jam. Tidak akan ada foto yang diambil dan tidak ada pula informasi personal yang akan kami kumpulkan dalam penelitian ini. Data yang anda berikan bersifat anonym sehingga tidak akan ada cara apapun bagi siapapun, termasuk kami, untuk mengetahui bahwa hasil kuesioner dan hasil wawancara tersebut adalah isian atau jawaban anda. Selain itu, anda juga berhak untuk tidak menjawab pertanyaan baik pertanyaan tertentu dalam wawancara maupun dalam kuesioner.
Bagaimana Informasi yang Anda Berikan dijaga Kerahasiaannya?
Setelah anda diwawancarai dan mengisi kuesioner, data yang anda berikan akan kami transkripsi dan kami berikan nomor kode. Petunjuk tertentu seperti nama, lembaga, dan sebagainya yang disebutkan dalam wawancara akan diblokhitam. Salinan hasil kuesioner dan wawancara akan kami simpan di dalam lemari terkunci. Siapapun yang membaca atau mendengarkan laporan hasil penelitian kami tidak akan mampu mengidentifikasi anda karena kami telah merangkum semua hasil wawancara dan kuesioner anda dengan partisipan lainnya dalam penelitian ini, sebelum semua fakta dibeberkan sebagai hasil analisis.
Apa Risiko Ikut Serta dalam Penelitian ini?
Tidak ada risiko fisik yang anda peroleh dari penelitian ini. Tidak ada pula risiko informasi maupun risiko kesakitan yang dapat muncul akibat partisipasi dalam penelitian ini.
Apa Manfaat Ikut Serta dalam Penelitian ini?
Setelah rangkuman kuesioner diperoleh, anda akan dapat melihat gambaran keseluruhan dari pengetahuan yang dimiliki pustakawan yang lain dan anda dapat membandingkannya dengan yang anda miliki. Manfaat utama dari penelitian ini juga ada pada komunitas pustakawan secara keseluruhan karena membantu dalam berbagi pengetahuan. Lebih dari itu, penelitian ini dapat menghasilkan system informasi yang dapat memudahkan pustakawan dalam bekerja di kemudian hari dalam menangani masalah-masalah pekerjaan sebagai pustakawan.
Apakah ada Biaya atau Pembayaran yang terlibat?
Tidak ada biaya yang perlu anda keluarkan untuk ikut serta dalam penelitian ini. Jika memang ada biaya yang tidak sengaja dikeluarkan, seperti anda harus ketinggalan kereta, biaya bahan bakar, biaya konsumsi, dan sebagainya, maka biaya-biaya ini akan dibebankan kepada kami dan dibayarkan setelah pengumpulan data selesai.

Walau demikian, jika berdasarkan diskusi, kami merasa bahwa data yang anda sediakan tersebut dapat digunakan untuk penelitian lainnya seperti penelitian lanjutan, artikel jurnal, atau prosiding konferensi, kami akan meminta izin Kembali kepada anda untuk tidak memusnahkan dan menggunakan kembali data tersebut.

Tacit Knowledge as an Enhancement of Business Intelligence Framework Using Cognitive-based Approach.
I, Herison Surbakti, a Ph.D student in information systems from Universiti Utara Malaysia, are conducting research on capturing the tacit knowledge framework for the business intelligence purpose. Please read this file in its entirety. We want to interview and ask you as a librarian to fill out the questionnaire in this study. We expect the participation of librarians to obtain the data that can be used for input into the business intelligence system that I design.
What is the Main Reason to Conduct This Research?
The research purpose is because it is very difficult for current organizations to manage knowledge specially to obtain tacit knowledge. This knowledge is difficult to capture and formalize. We designed a method that could capture the tacit knowledge through ongoing interviews and questionnaire phases on librarian as the samples that were deemed experienced. This interviews and questionnaire will provide tacit knowledge data which will then be processed by system that will develop and become a test of whether the framework that we create is able to automatically process the knowledge of the tacit.
What Are Involve in This Research?
If you choose to participate in this study, we will ask you to do a number of things. First of all, we will interview you about the facts of your work as a librarian. After this interview is complete, we will compile a questionnaire of all answers given by all librarian participants.
Next, we will come to you again to conduct the second stage of interview while submitting a questionnaire for you to fill out. This second stage interview aims to deepen the answers you have given in the first stage. In the last section, we ask that you mention what are the difficulties you have faced to say in an interview because it can only be demonstrated through actions. If there is, then we will ask you to recommend one colleague who has observed you in the past in carried out some problems at work and we will ask your permission to interview your co-worker to understand your difficulties in saying. We will ask you to sign a form of paper to allow your co-worker to provide this information.
Meanwhile, the questionnaire aims to ask you to rank a number of aspects based on
frequency, level of difficulty, efficiency, or the possibility of a number of things that have been obtained from the first interview.
After this process, we will analyze the results of questionnaire entries and make the basic statistics for each answer. We then contact you again for the third phase of the interview, which is the last stage. The purpose of the third phase of the interview is to ask you to review the results of the collective questionnaire, listen to your comments about the results, and ask that you consider changing or not changing your answers to the questionnaire after seeing the results of the collective questionnaire. If you also recommend someone as a co-worker in the second stage of the interview, then at this stage we will also interview the colleague that you have recommended.
Each interview and filling out the questionnaire will only take one hour approximately.
Therefore, in total, your direct participation in this study is three hours approximately. There will be no photos will be taken and no personal information that we will collect in this study.
The data you provide is anonymous so that there will be no way for anyone, including us, to know that the results of the questionnaire and the results of the interviews are your entries or answers. In addition, you also have the right not to answer questions either specific questions in interviews or in questionnaires.
How the Information You Provide Will Keep Confidential?
After you have been interviewed and filled out the questionnaire, the data you provide will be transcription and we will provide the code number. Certain instructions such as names, institutions, and so on that have been mentioned in the interview will be blocked black. We will keep a copy of the results of the questionnaire and interview in a locked cabinet.
Anyone who reads or listens to our research report will not be able to identify you because we have summarized all the results of your interviews and questionnaires with other participants in this study, before all the facts are revealed as a result of the analysis.
What Are the Risks of Participating in This Study?
There are no physical risks that you will have from this study. There is also no information risk or risk of pain that can arise due to participation in this study.
What Are the Benefits of Participating in This Study?
After the summary of the questionnaire has been obtained, you will be able to see the overall pictures of the knowledge that librarians have and you can compare it with what you have.
The main benefits of this study also exist in the librarian community as a whole because it helps in sharing knowledge. More than that, this research can produce information systems that can facilitate librarians in working later in handling work problems as librarians.
Are There Fees or Payments Involve?
There are no fees that you need to spend to participate in this research. If indeed there are unintentional costs incurred, such as you have to miss the train, fuel costs, consumption costs, etc., these fees will be charged to us and paid after the data collection is complete.

What Happens to The Data I Will Give After This Research is Complete?
After this research is complete, we will burn all the questionnaires and interview transcripts and delete the soft files from the questionnaire and interview records from the computer so there is no possibility that the data will be used for other possibilities. However, if based on discussion, we feel that the data that you provide can be used for other research such as advanced research, journal articles, or conference proceedings, we will ask your permission to detain the destroying and reuse the data.
What is Your Right as a Participant?
You are free to be involved or not be involved in this research. There is no penalty or loss of benefits if you decide not to participate. If you decide to participate, you can withdraw/cancel participation whenever you want. You have the right to request that we
destroy or not destroy the data that you provided after the study was completed. You will be given a copy of this file for storage.
